(question) Overview

Anonymous forms allows you to share a link to a form with someone who does not have a ShareVision user account so that they can fill in the form without having to login to your ShareVision site. You can either email the link to them or make the Once Anonymous Forms is configured, a link to the form available to them (i.e. publish it can be copied and shared. The link can be emailed or published on your website). When the link is selected, the form opens in a browser window and the form can be filled in and saved to your ShareVision site.

Note: The person filling in the form is not prompted to login in order to access and fill in the form. The Created By field on any form that is filled in using this feature will be set to

ShareVision Anonymous instead of the name of a ShareVision user. 

(blue star) Use Case

Customers have used this add-on in the following scenarios:

  • Referral Form: Add a link to a Referral Form for their organization. Anyone who visits their website can fill in the form. If the referred individual client is accepted and registered into their organization, the referral form can be linked to the Individual's Details in ShareVision. Intake Form(s): An individual who has been accepted as a new client in an organization is emailed a link to the Intake Form(s) and fills in the form(s). The form(s) can be linked to the Individual's Details in ShareVision or the information captured in the form(s) can be used to populate other ShareVision forms's details in ShareVision

  • Employment Application Form: An organization adds a link to an employment application form on their website. The submitted application form can be reviewed by the hiring manager in ShareVision.

  • Activity Sign Up Form: A link to the Activity Sign Up Form is either emailed to an Individual a client or is added to the organization's website and the individual client can sign up for an activity using the anonymous form. The form can be linked to the Individual's Details client’s details in ShareVision.

Recommendation

If you are using Use this feature so that Clients clients you support can fill in the form using a link on your website or that you send to them via email, it is recommended that you add a column as a single line of text to the form to capture the Individualclient's name.

We do We do not recommend including the Individual Client column on the form as this will create a drop-down list that will display all Clients on your ShareVision site to anyone that is completing the form.

Please be sure to review the information in the Additional Setup Information section at the bottom of this article prior to setting up an Anonymous Form on to learn more about the two methods your ShareVision site can be set up when using this add-on feature. Each method supports linking an Anonymous Form to an Individual after the form has been completed and submitted.

How to Access Forms


Once you have identified which form on your site that you would like to set up as an Anonymous form, you will need to edit the form.

  1. Login to ShareVision with a user account that has Site Administrator permissions

  2. Select (click on) your name in the upper right hand corner

  1. Select Site Administration on the menu that appears

  1. The Site Administration Home page will be displayed. Select Forms in the Site Content section:

How to Set Up an Anonymous Form


  1. In the search bar at the top of the Forms window, search for the form. In this example, we will type “referral” to search for the Agency Referral form

  1. Select the three dots on the left side of the Agency Referral form that will be displayed in the search results and select Edit Form

  1. Select the arrow next to the Advanced Settings section to expand this section

  1. Check the box next to Allow this form to be accessed anonymously to enable the anonymous form

  1. After enabling the anonymous form, you will need to Choose where users will be taken after completing the form. There are 2 options:

    1. Custom landing page

    2. Redirect to another site

  1. For any form that has Anonymous forms enabled, a form icon will appear next to the Advanced Settings section for the form

  1. The forms icon will also appear next to the form name when viewing the list of forms on your site

About the Custom Landing Page Option

If you select the Custom Landing Page, you will need to set up a message that will appear after the form has been filled in and saved

  1. To add the message that will appear after the form is filled in and saved, create the message in the Custom landing page field that appears

  1. Save the form

About the Redirect to Another Site Option

If you select the Redirect to Another Site option, you will need to enter the Site URL (web page address) that will appear after the form has been filled in and saved

  1. To add the Site URL that will appear after the form is filled in and saved, enter or paste the web page address in the Site URL field that appears

  1. Save the form

Previewing an Anonymous Form


You can preview the anonymous form to see how it works.

  1. On a form where anonymous forms has been enabled, expand the Advanced Settings section

  2. Select the Copy Link button

  3. A message will appear to confirm that the link has been copied. The link to the anonymous form is saved to your computer’s clipboard. It can be pasted as a link in an email or you can use this link to create a link to the form from your website. Select OK.

  4. Open a browser window in incognito (Chrome) or InPrivate (Edge)

  1. Fill in the form and select save in the upper right hand corner

  1. Either the Custom Landing Page or the web page for the Site URL you entered will be displayed

  2. You can close the tab

Using an Anonymous Form


When you are ready to start using the anonymous form, you will need to share the link so that it can be filled in and saved.

  1. On a form where anonymous forms has been enabled, expand the Advanced Settings section

  2. Select the Copy Link button

  3. A message will appear to confirm that the link has been copied. The link to the anonymous form is saved to your computer’s clipboard. It can be pasted as a link in an email or you can use this link to create a link to the form from your website. Select OK.

  1. When the link is selected, the form will open and it can be filled in and saved.

Viewing Submitted Forms


If you have already set this form up in Portal Settings, you can navigate to the Portal and Page where it was published using the Quick Start Menu. If you haven’t yet set the form up in Portal Settings, please review the information about Portal Settings.

Alternatively, you can use the Add Form to Portal feature.

Viewing Form Data


You can also view the data collected for forms that have been submitted. Please review View Form Data for more information.
