Overview


The extended form features add-on gives you more control over how your forms are presented to the user by allowing you to create several new types of columns. By using "delimiters" or special characters along with keywords in the column name, this add-on can create 3 different types of form groups that will show based on a choice or expand and collapse. Use the following simple instructions to get started.


Feature 1: Choose Type Section

Administrators

Part 1: Create a Type section heading

  1. Create a new column with the title Type: Colon or other delimiter* is required.
  2. Make this a Choice column. Enter the choices below.
  3. Display choices using radio buttons.
  4. Ensure Default value is set to Choice. Leave the default value field blank.

  

Part 2: Create section questions

  • For each section question, create a new column with the format choice name: question

Staff

  • Select a Type to unlock related questions.

 


  

Feature 2: Expand/Collapse Section

Administrators


Part 1: Create an expand/collapse section heading

  1. Create a new column with any title followed by a colon. Colon or other
    delimiter* is required.
  2. Make this a Yes/no column.


Part 2: Create section questions

  • For each section question, create a new column with the format section heading name: question
 
        Colon or other delimiter* is required. Example seen on left.

Staff

  • Section headers expand to reveal questions. Collapse sections to hide questions.

Feature 3: Applicable Questions Section

Administrators


Part 1: Create an applicable-questions section heading

  1. Create a new column with any title followed by "section:" or (-) delimiter, as seen in the example seen on left, "Persons Notified Section:"
  2. Make this a Yes/No column, with the default set to "No".

Part 2: Create section questions

  • For each section question, create a new column with the format "section heading name: question". i.e. "Persons Notified Section: Police?


Staff

  • Check to open section, if applicable. Uncheck to remove all section data.


On a New Form

*Delimiters activate the Extended Form Features Add-on. Delimiter choices include a colon (:) or a hyphen (-).
